Official Terms and Conditions of the “CARE BONUS” Promotion at Porcelain Dent

1. General Provisions

1.1. The “Care Bonus” promotion (hereinafter — Promotion) is conducted by Porcelain Dent dental clinic (hereinafter — Organizer) to encourage patients to attend preventive and treatment visits.
1.2. The Promotion is valid at all Porcelain Dent locations from November 26 to November 29, 2025, inclusive.

2. Participants of the Promotion

2.1. Any individual may participate in the Promotion if they:

- visited a Porcelain Dent clinic during the Promotion period;
- paid for dental services totaling at least ₴500;
- provided accurate contact information (full name and phone number).

2.2. Clinic employees and their family members may receive treatment but are not eligible to participate in the certificate raffle.

3. Promotion Mechanics

3.1. Every patient who pays for services during the Promotion period receives a cashback bonus, which can be used during their next visit.
3.2. The cashback amount depends on the total bill:

Check Amount - Cashback Bonus
500 – 4,999 UAH - 500 UAH
5,000 – 19,999 UAH - 1,000 UAH
20,000 – 99,999 UAH - 2,000 UAH
100,000 UAH and more - 5,000 UAH

3.3. Cashback is credited after full payment for services.
3.4. Cashback is valid for the next visit and can be used for any dental service (except the purchase of oral care products, promotional offers, or gift certificates) starting December 1, 2025.
3.5. Cashback validity period — unlimited.
3.6. Cashback cannot be exchanged for cash.

4. Certificate Raffle

4.1. Among all patients who visit the clinic from November 26 to November 29, 2025, a raffle will be held for a certificate for a professional oral hygiene procedure.
4.2. No additional actions are required to participate — simply being a patient during the Promotion period is enough.
4.3. The winner will be selected randomly among all participants on December 2, 2025, using an open online tool (e.g., random.org).
4.4. The raffle results will be published on the official Porcelain Dent pages on Instagram and Facebook.
4.5. The winner will also receive a personal message or a phone call from a clinic administrator.
4.6. The certificate is non-refundable, non-exchangeable for cash, and valid until December 1, 2026.

5. Use of Personal Data

5.1. Participation in the Promotion constitutes consent from the participant to the processing of their personal data (name, phone number, email) solely for the purposes of participation and subsequent communication about clinic services.
5.2. The Organizer guarantees the confidentiality of all collected information in accordance with the applicable laws of Ukraine.

6. Other Terms

6.1. The Organizer reserves the right to change the Promotion terms, duration, or terminate it early by posting a corresponding notice on the official clinic website.
6.2. Any disputes shall be resolved through negotiations between the Participant and the Organizer.
6.3. Participation in the Promotion confirms that the Participant has read and agrees to these Terms and Conditions.
